What to Expect Before, During, and After Sinus Surgery

September 30, 2025

Chronic sinusitis is more than just a stubborn cold. For over 28.9 million American adults diagnosed with this condition, it can mean relentless facial pressure, debilitating headaches, and constant fatigue. When medications and rinses no longer provide relief, sinus surgery can be a life-changing solution.

If your ENT has recommended Functional Endoscopic Sinus Surgery (FESS), it’s completely normal to have questions. This guide will walk you through the entire process—from preparation to recovery—to help you feel informed and confident about your journey to better breathing. With insights from our board-certified experts, Dr. Alexander Farag (Neurorhinology, Advanced Rhinology, and Skullbase Surgery), and Dr. Paul Radabaugh (Neurorhinology, Advanced Rhinology, and Skullbase Surgery), you can trust you're getting expert advice.

Why is Sinus Surgery Recommended?

Sinus surgery isn’t the first option; it’s typically considered only after other treatments have failed. The most common procedure, FESS, aims to fix the "plumbing" of your sinuses. The goal is to:

- Open blocked sinus pathways

- Remove polyps or diseased tissue

- Restore natural drainage and airflow

- Improve your ability to breathe and sense of smell

Phase 1: Preparing for Sinus Surgery – What You Need to Know

Preparation is the foundation for a successful outcome. This phase is all about planning and communication with your medical team.

The Pre-Surgery Checklist:

- Confirmation & Mapping: Your sinus surgery journey starts with a thorough evaluation. After a physical exam and an in-office nasal endoscopy, we confirm the diagnosis. This almost always includes a CT scan to create a detailed, anatomical roadmap of your sinuses, allowing us to develop a tailored, precise surgical plan.

- Medication Review: You will be provided with a list of medications and supplements (like aspirin or ibuprofen) to avoid, as they can increase bleeding risk.

- Home Prep: Make sure to set up a comfortable recovery space at home. Stock up on easy-to-prepare meals, saline rinse kits, and any prescribed medications beforehand.

Phase 2: What to Expect During Functional Endoscopic Sinus Surgery (FESS)

The word "surgery" can sound intimidating, but FESS is a minimally invasive, outpatient procedure. This means you go home the same day.

Key Things to Know:

- No External Scars: The entire surgery is performed through your nostrils using a small, high-definition camera called an endoscope.

- It’s Not a "Nose Job": FESS focuses entirely on your sinuses and does not change the external appearance of your nose.

- Precision is Key: Using specialized instruments, your surgeon will carefully clear blockages and widen sinus openings to restore function.

- Advanced Comfort: Many surgeons now use dissolvable dressings, some of which slowly release medication, making recovery much more comfortable than older packing methods.

Phase 3: Postoperative Care – Your Role in Recovery

Your active participation is the most important factor for a smooth recovery. Here’s what to expect.

The First 48 Hours:

- Rest is Best: Plan to rest with your head elevated to minimize swelling.

- Normal Symptoms: Some bloody drainage and nasal fullness are completely normal.

- Manage Discomfort: Use prescribed pain medication as needed.

The First Week & Beyond:

- Saline Rinses are Essential: This is the #1 thing you can do for success. Rinses keep the healing passages clean, moist, and clear of debris.

- Follow-Up Appointments: You’ll have post-op visits for in-office debridement. These quick cleanings are crucial to prevent scarring and ensure everything is healing properly.

- Patience Pays Off: While you may feel better quickly, full healing takes time. Most patients experience a significant, lasting improvement in their symptoms and quality of life.

Oak trees are abundant in Jacksonville and the surrounding areas, providing great shade and beauty. However, during their peak season from March to June, these trees release copious amounts of lightweight pollen into the air to fertilize other trees. This process can lead to high pollen counts that easily travel long distances, making it simple for pollen to enter your home and sinuses. For allergy sufferers, exposure can trigger a range of allergic reactions.
